She would be eligible for both Medicaid and Medicare after being a permanent legal resident of the USA for 5 years; she doesn't need to be a citizen. For Medicare she would have to pay both Part A and Part B premiums.

There are few plans available to anyone over 65. Those that are available are more lenient with regards to pre-existing conditions because they won't cover those conditions for 24 months. Call a local agent to see what plans may be available in her area. Most agents don't work in this market so you may have to make many calls before you can find someone knowledgeable.

First, it will be good to get a Summary of Benefits on your employer plan. It is usually available once you become eligible for the plan and outlines what coverage you have for clinic and hospital services, tests and procedures, what to do in case of emergency, etc. Once you get the summary, if you read over the material and don't understand it, just call your insurance customer service rep for clarification. If your plan requires that you use a specific network of doctors, pharmacies and other providers, then the same customer service rep should be able to help you in determining where it's most convenient to establish your medical care. If you are due to have a medical exam (perhaps to renew medication or continue treatment on an existing condition) then it would be good to get established with a doctor recommended by your plan. The doctor may want your past medical records to continue your care and his/her office staff can help you in getting them. These programs may provide low-cost health coverage for children and teens up to age 19 including comprehensive health, dental and vision coverage. Moving the family's young dependents from private insurance to government sponsored plans can reduce monthly premiums and perhaps make a family's adult private premium coverage affordable.
